Mannington Township School's Health Office
The Health Office is open daily from 8:00 a.m. to 3:20 p.m. during the school year.
The school nurse, Mrs. Suzann Mendenhall, may be reached at (856) 935-1078, ext. 25. There is a voice mail answering service for messages if the nurse is unavailable. Please feel free to leave a message and your call will be returned as soon as possible.
Immunization Requirements
The following are the minimum immunization requirements for entry into grade level as established by the New Jersey State Department of Health:

| Measles | 1 dose on or after the child's 1st birthday. |
| Mumps | 1 dose on or after the child's 1st birthday. |

At least 1 dose on or after the child's 1st birthday. |
Kindergarten |

3 doses with a booster after 4th birthday (max. of 5 fulfills requirement). |
Polio |
2 doses with booster after 4th birthday (max. of 4 fulfills requirement). |
| Measles | 2 doses, the first on or after child's 1st birthday & the second no less than one onth after the first dose. |
| Mumps | 1 dose on or after the child's 1st birthday. |
| Rubella | 1 dose on or after the child's 1st birthday. |
Hepatitis B |
3 doses. |
| Varicella (Chicken Pox) | 1 dose on or after the child's 1st birthday or documented laboratory evidence, a physician's statement, or a parental statement of previous varicella disease. |
| Grade 6 | |
| Hepatitis B | 3 doses, if not received at a younger age |
The only exceptions to these requirements are medical or religious exemptions.
