New Student Health Information

WELCOME
I would like to take a moment to introduce myself and to welcome you and your child to Mannington Township School. As a parent we are apprehensive when sending our young ones off to school for the first time, I too once shared these feelings. However, I would like to insure you that as a mother and nurse your child is in a safe, secure, and loving environment. If your child is injured or becomes ill during school hours you will be notified by phone or a written letter. Please inform me of any pertinent medical history or significant information that is needed to treat your child appropriately. I have an open door policy to students, teachers, staff and most of all parents. Please let me know if you have any questions about the health of your child or about school health policies. I wish you and your family a healthy and safe school year.
HEALTH OFFICE REQUIREMENTS
All new students must provide a copy of an updated immunization record. Please refer to the Immunization requirement link under the Health Forms tab. As a result of a change in the law and the rules of the New Jersey State Board of Education and the New Jersey Department of Health and Senior Services, parents are now required to obtain school physical examinations for their children at their "medical home" instead of these physicals being provided by the school district. The medical home is your own personal family physician, doctor of your choice, or nurse practitioner. The cost of the physical is the responsibility of the parent/guardian. Physical examinations are required for the following students:
- All students entering Pre K (must be completed and the form returned before entering school
- All new students entering Kindergarten who did not attend Mannington School's Pre K (must be completed and the form returned before entering school)
- All new students entering from out of state
- Some students as referred by the Child Study Team
Physicals listed above must be completed within 365 days prior to school entry. The examination reports must be recorded on the form with this informational letter. In addition, it is strongly recommended, but not required, that you obtain a physical examination for your child during the following developmental stages:
- at least one time during preschool to 3rd grade
- at least one time during 4th to 6th grade
- at least one time during 7th to 12th
